Reporting Absences (Short-term)

For short-term absences (fewer than four days), please use the form below when your son or daughter will be:

1. Absent,

2. Late, or

3. Needs to leave school early. Kindly give us sufficient notice.

If more than one child is away from your family, please fill in a separate form for each child.
